Dear VA Director Gardner:

Would you please explain the "Request for Information" letter dated June 3, 2015?

(1) What triggered the letter? Why now?

(2) Who is the sender?

(3) Why would the VA not know that the only benefit I receive is the economic pension for veterans over 65 who live under the poverty line and that I have no applications outstanding? The avalanche of confusion and misinformation in this letter covers this up.

(4) Why the false statements on page 2 under "We need to verify ..." ?

(5) Why the "You must give us the information no later than 06/18/2015"?

(6) Why the "If we do not get the information and we do not hear from you, we will deny your application or stop/change your benefits"?

(7) Why am I being made to appear responsible?

(8) Is this letter a set up to take away my VA economic pension and make it appear as my fault? This would drop my income from $12,876 to $6,792. When my medicine is stopped I won't be able to buy it from the private sector. Doesn't this kill me?

Why won't you allow me to get my 2003 Power of Attorney and Living Will out of the VA system? What is your intent?

I use Lorazepam for atrial fibulations. My past prescription was for 4 fills with 60 tablets per fill or 240 tablets total (2012, RX# 1964138A). Why can't I get more than 17 tablets total now?

Why the continued discrepancy between the number of fills stated on the medicine bottles and the number of fills I actually receive? I receive one less fill than it says on the bottle. The number of fills I actually receive matches the number of fills shown on the VA website http://www.myhealth.va.gov

Would you take an accountable position?
