Dear Acting Director Jennifer Gutowski:
Welcome to Tucson. I did not know that Director Johnathan Gardner had stepped down.
I don't understand your letter to me dated April 5, 2016:

"Dear Mr. O'Connell
Thank you for your letter to the Under Secretary for Health, regarding your health care concerns at the
Southern Arizona VA Health Care System (SAVAHCS), Your letter was forwarded to the Department of
Veterans Affairs (VA) and I am responding on behalf of the Department. I requested Anthony Stazzone, MD,

concerns.
After reviewing your correspondence, Dr. Stazzone reviewed your request for medication renewal. Your
Primary Care Provider, Dr Susan Morris, has refilled your current medications and will speak with you
regarding future renewals at your appointment on May 12, 2016, at 9:00am. Dr. Stazzone has provided his telephone number via securred messaging should you have any further questions.

concerned with patient care and the satisfaction of the Veterans we serve. If you have any further questions
or concerns about this letter, please contact Mr. Holmes at(520) 629-4950).

I don't remember sending a letter to the Under Secretary of Health. Please send me a copy.
Please expose the medicine trail for my Bupropion and show me what you see.

Dear Mr. OConnell:
In Reply, Refer To: 678/0-002
This letter is in response to your email inquiries dated April 9, 2016, April12, 2016 and April
14, 2016.
Your first inquiry was regarding the status of Mr. Jonathan Gardner, the previous Southern
Arizona VA Health Care System (SA V AHCS) Director. Mr. Gardner retired this past January
after 36 years of Federal service. I am currently serving as the Acting Director.
You also indicated that you do not remember sending a letter to the Under Secretary of Health
and requested a copy. Your inquiry to the Under Secretary of Health was in the form of an email
sent on April 1, 2016 at 1 0:36AM. I have included a copy of that email for your reference.
Lastly, you questioned your Bupropion medication. Your Bupropion prescription is current and
may be refilled using MyHealtheVet, or phone (1-800-470-8262). Dr. Morris has written you a
MyHealtheVet secure message with information regarding your refill, and will speak with you
regarding future renewals at your appointment on May 12,2016, at 9:00am.
Please use MyHealtheVet secure messaging, or postal mail, for any future concerns related to
your health care from the SA V AHCS. The emails you are sending to me are not a secure method
of communication. You can contact your PACT team directly through either MyHealtheVet
secure messaging, or postal mail. Your PACT team consists of Dr. Susan Morris, MD, Michelle
Treen, RN, Marci Johnson, LPN, and Michael Bernard, Medical Support Assistant. Secure
messaging with your PACT team is the best method to access the status of your health care.
If you have future concerns that your PACT team is unable to address, please contact Ms. Teresa
Cox, Administrative Officer for the Assistant Director, at (520) 792-1450 x6571, or by postal
mail at: Southern Arizona VA Health Care System (0-002), 3601 S. 6th Avenue, Tucson, AZ
85723.
Sincerely,
'fer S. utowski, MHA, FACHE

Arizona Department of Veterans Service (AZDVS) references it twice, and the Saint Paul VA in
Minnesota references it.
